I. INTRODUCTION
This volume is the fifty-ninth annual vital statistics report of
the State of North Carolina.
The computer information system underlying the automated report is
depicted in the flow diagram on the previous page. The computer-based system
has resulted in increased flexibility in information retrieval and more timely
dissemination of vital statistics. This system usually permits publication of
vital statistics during the month of April in the following calendar year.
This report shows annual frequencies and rates and five-year rates
for total, white and nonwhite selected vital events by each county and major
incorporated city in North Carolina. The vital statistics for a city, a
county or the State are together in one table on a single page. The cities
and counties are in alphabetical sequence. Trends in State data are discussed
in Section II
.
Beginning with the years 1971-73, the publication North Carolina
Vital Statistics, Volume 2 examines leading causes of death for counties, four
regions and the State. This volume, which includes mapping of actual and age-race-
sex-adjusted county rates for total mortality, infant mortality and 25
specific causes of mortality, will be repeated every three years. In addition,
interim reports of tabular data will be prepared on an annual basis. An
auxiliary report entitled Detailed Mortality Statistics enumerates State and
county annual deaths classified according to the specific causes of death and
the age, race and sex of decedents. We hope that together these reports will
serve your needs.

II. TRENDS IN VITAL STATISTICS
North Carolina's population and vital record registrations in recent
years reflect the following trends:

Expanded analyses of these trends follow. These analyses are based
on annual vital record registrations and population bases applicable to July 1
of the registration year. The population bases for 1973 and 1974 are described in
Section V. For all other years 1960-1972, population bases represent straight-line
interpolations or extrapolations of the 1960 and 1970 Censuses (1, 2). The
reader will note that alternate bases for 1971 and 1972, described in Section V,
were used in the calculation of the five-year rates of this report. The
straight-line extrapolations were maintained for the trend analyses in order
that rates conform exactly to those depicted in the 1971 and 1972 annual vital
statistics reports.
In the analyses which follow, the several statistics are discussed in
the order of their appearance in the tables of this report. Counties and cities
may review their respective vital statistics trends in order to detect departures
from the State trends. For definitions and formulas, see Section VII.

Population : NET MIGRATION "IN" FOR WHITES, "OUT" FOR NONWHITES
Between 1960 and 1970, the North Carolina population increased by
526,000 persons, an 11.5% increase over the 1960 Census. The nationwide
increase was slightly higher, 13.3%. North Carolina whites increased by 14%
and nonwhites by 3%. The urban population increased by 27% while the rural
population increased by 1.5%. (2)
Both whites and nonwhites experienced net outmigration during the
nineteen-fifties, but whites netted an in-migration of 97,950 persons (+2.9%)
between 1960 and 1970. Outmigration of nonwhites remained high. The net was
170,752 persons (-14.8%).
In-migration of whites and outmigration of nonwhites lowered the
percentage of nonwhites in the population from 25.4 in 1960 (1) to 23.2 in
1970 (2). The reduction has been delimited by a higher rate of natural
increase among nonwhites. In 1974, natural increase per 1,000 population was
5.7 for whites, 12.0 for nonwhites and 7.1 for all races combined.
Live Births : CRUDE BIRTH RATE LOWEST EVER; ILLEGITIMACY CONTINUES UPWARD
The North Carolina birth rate was 15.7 resident births per 1,000
population in 1974. This was the lowest birth rate ever recorded in North
Carolina, down 19 percent since 1970 when the rate was 19.3. The white birth
rate was 17.8 in 1970 compared to 13.9 in 1974. For nonwhites, the rate was
24.4 in 1970 and 21.9 in 1974.
The percentage of resident live births attended by a physician was
99.6 in 1974 compared to 99.1 in 1970. The trend for hospital births has also
been upward, from 98.5 percent in 1970 to 99.1 percent in 1974.

The percentage of births classified as premature, 5 pounds 8
ounces or less, was 8.6 in 1974 compared to 8.7 in 1973. The percentages were
8.8 in 1960 and 9.2 in 1970. The nonwhite premature rate is nearly two times
the white premature rate.
The percentage of births classified as illegitimate was 15.0 in 1974
compared to 14.7 in 1973 and 12.6 in 1970. The nonwhite illegitimacy rate is
nearly ten times the white illegitimacy rate.
Fetal and Infant Mortality : INFANT MORTALITY REDUCED BY 20% SINCE 1970;
NONWHITE POSTNEONATAL MORTALITY DOWN BY 31%
In 1974 the North Carolina perinatal mortality rate was 28.0 resident
deaths per 1,000 deliveries. The rate continued a declining trend in perinatal
mortality. The perinatal mortality rate was 30.6 in 1973 and 33.8 in 1970.
Reductions in perinatal mortality have occurred primarily in the nonwhite segment
of the population.
The North Carolina fetal mortality rate was 14.1 deaths per 1,000
deliveries in 1974 compared to 15.2 in 1973 and 14.8 in 1972. The white fetal
mortality rate was 11.2 in 1974; the nonwhite rate was 20.5. Comparisons to
fetal mortality rates observed prior to 1972 are precluded by 1971 changes in
the North Carolina abortion law (G.S. 14-45.1) whereby fetal deaths of 20 or
more weeks gestation may be reportable as therapeutic abortions.
The North Carolina neonatal mortality rate was 14.1 deaths per 1,000
live births in 1974. In 1970, the neonatal mortality rate was 17.9. The white
neonatal mortality rate was 15.3 in 1970 and 12.3 in 1974. The nonwhite neonatal
mortality rate was 24.1 in 1970 compared to 17.8 in 1974.

The North Carolina postneonatal mortality rate was 5.4 deaths per
1,000 neonatal survivors in 1974. A downward trend in postneonatal mortality
gained impetus in 1967. The white postneonatal mortality rate was 4.0 in
1970, 3.9 in 1971, increased to 4.5 in 1973 and dropped to 4.1 in 1974. The
nonwhite postneonatal mortality rate declined from 12.0 in 1970 to 11.0 in
1971, increased to 11.5 in 1972 and dropped to 8.3 in 1974.
The North Carolina infant mortality rate in 1974 was 19.4 deaths
per 1,000 live births. This rate represents a decrease of 20 percent since
1970 when the rate was 24.1. The white infant mortality rate was 19.2 in
1970 and 16.4 in 1974. The nonwhite infant mortality rate was 35.8 in 1970
and 26.0 in 1974. The United States also continues to experience reductions
in infant mortality (3).
Deaths : DEATH RATE DROPS; EXCESSIVE MALE AND NONWHITE MORTALITY CONTINUES
The North Carolina death rate was 8.6 resident deaths per 1,000
population in 1974, down 5 percent from a rate of 9.1 recorded in 1972 and
1973. The male rate of 10.1 compares to a female rate of 7.2. The death
rate was 21 percent higher for nonwhites than for whites. The white and
nonwhite death rates were 8.2 and 9.9 respectively. Both sexes and races
contributed substantially to the five percent decline between 1973 and 1974.
For all age categories, the nonwhite rate was higher than the
white rate. A nonwhite child 0-4 years of age was nearly twice as likely
to die as a white child of the same age. A nonwhite adult 25-44 years of
age was nearly three times as likely to die as the white adult in the age
group.

Approximately three out of five resident deaths occurred in a
hospital in 1974. Less than four percent of the deaths which occurred in
North Carolina were to nonresidents of the State.
Other Events : MARRIAGE RATE DECLINES; DIVORCE RATE CONTINUES UPWARD
The North Carolina marriage rate declined in 1974 to 8.4 marriages
per 1,000 population. The rate was 9.0 in 1973 and 9.4 in 1972. In 1962,
when marriage registration began, the rate was 7.3. The provisional United
States rate of 10.5 in 1974 represented the first national decline since
1958 (3).
The North Carolina divorce rate was 3.7 divorces per 1,000 population
in 1974, up 37 percent from a rate of 2.7 in 1970. The rising divorce rate
for the United States was estimated at 4.6 in 1974 (3).
The North Carolina divorce rate accelerated after 1964 when one-year
separations replaced two-year separations as grounds for divorce. In the
United States, the divorce rate began rising in 1963 (3).

III. REGISTRATION OF VITAL EVENTS
The origin, flow and disposition of North Carolina vital records are
illustrated in the flow diagram on page 1-8. A brief history of vital records
processing in North Carolina and current registration practice are discussed
below. The user of this report may note in particular the section regarding
registration of fetal deaths. Legislation enacted by the 1971 North Carolina
General Assembly has altered fetal death registration practice.
Birth and Death
The Bureau of Vital Statistics, with the State Health Director as
State Registrar of Vital Statistics, was created by an act of the General
Assembly of 1913. The 1973 General Assembly changed the name of the office
to the Department of Human Resources and made the Secretary of Human Resources
the State Registrar of Vital Statistics. The authority of this department
and the laws which it administers are contained in Chapter 130, Article 7,
of the General Statutes of North Carolina. Within the organizational struc-ture
of the Department of Human Resources, the duties of the State Registrar
of Vital Statistics are accomplished primarily by the Vital Records Branch
with support from the Public Health Statistics Branch.
The collection of birth and death records began on October 1, 1913.
The Department of Human Resources is the custodian of these records. Statutes
enacted by the General Assembly of 1913 require that physicians, midwives, or
other persons attending a birth file a birth certificate within five days
after birth. The funeral director or person burying a body is responsible
for filing a death certificate or fetal death certificate with the local
registrar prior to final disposition and within 72 hours after death.

The local registrar is the health director in 97 of 100 counties.
A clerk of the county health department, designated by the local registrar
as deputy registrar, carries out registration procedures locally. The
deputy registrar forwards the original birth and death certificate to the
Department of Human Resources on the fifth day of the month following the
event, where they are processed and permanently preserved. Periodically,
birth and death certificates are microfilmed and sent to the National Center
for Health Statistics.
North Carolina was admitted into the National Death Registration
Area in 1916 and into the National Birth Registration Area in 1917.
Fetal Death
Prior to May 19, 1971, North Carolina law required the filing of
a fetal death certificate for fetal deaths (stillbirths) of 20 or more weeks
gestation. Legislation enacted by the 1971 General Assembly (G.S. 14-45.1)
requires that therapeutic abortions be reported separately. For fetal deaths
falling within the purview of the referenced statute, the therapeutic abortion
report form now serves in lieu of a fetal death certificate. For this reason,
counts of fetal deaths and perinatal deaths in 1971 through 1974 are not
strictly comparable to counts in previous years. In 1974, approximately 288
fetal deaths of 20 or more weeks gestation were preferentially reported as
therapeutic abortions.
Marriage and Divorce
In 1957, the General Assembly enacted legislation requiring that
the duties of the clerk of court include the registration of divorces and

annulments of marriage effective January 1, 1958. Lists of divorces and
annulments are required to be sent to the Department of Human Resources on
or before the 15th day of the month following the event.
By virtue of legislation enacted in 1961, the duties of the county
register of deeds include the registration of marriages occurring on or
after January 1, 1962. Copies of the certificates of marriage are required
to be forwarded to the Department of Human Resources on or before the 15th
day of the month following the marriage. The original certificates are
filed with the county registers of deeds.
In January 1964, North Carolina was admitted to the National
Marriage Registration Area. North Carolina is not a member of the National
Divorce Registration Area.

IV. TECHNICAL NOTES
Completeness of Registration of Vital Events
Completeness of registration is defined as the degree to which
reported vital event enumerations agree with the true counts in a given
time frame. Birth registration, tested for completeness in 1940 and 1950,
was found to be 86 and 96 percent complete respectively (4). Death regis-tration
is believed to be even more complete than birth registration.
Marriage registration was tested for completeness in 1963. Over
99 percent of performed marriages terminated in properly recorded certifi-cates
of marriage (5).
Delayed registration results in underenumeration of vital events
for a specific time period. Frequency counts in this report are based on
1974 events filed with the local registrar prior to March 1, 1975 and
processed in the State office before the end of March 1975. Based on
examinations of lagtimes in prior years, the number of calendar year events
filed after March 1 of the subsequent year is considered negligible. As this
publication goes to press, 19 birth and 19 death records for 1974 were pro-cessed
during April and not added to the statistical file. These residuals
are considered amongst the 0.2 - 0.5% which will accrue during 1975.

Accuracy of Items on Certificates
The Vital Records Branch maintains continuous surveillance of
certificates to detect inconsistencies in related items (e.g., newborn's
name vs. sex). However, the true extent of bias and unreliability due to
incorrect answers is not known.
Treatment of Missing Values
As a result of a query program to reconcile inconsistencies in
the data and to complete unanswered items, missing values are rare for the
characteristics included in this report. These "not stated" cases are
treated in one of two ways:
(1) not allocated because they are believed to have
an unknown distribution which is different from
the distribution for "stated" cases;
(2) randomly distributed in the same proportion as
the "stated" cases for the State.
The following table shows the number and treatment of missing
values for the statistics found in the detailed tables.

Residence Allocation
Except as otherwise indicated, vital events in this report are
allocated to the place of residence (county or city) regardless of the
place of occurrence. In the case of a birth or fetal death, residence is
the usual residence of the mother. Deaths are allocated to the residence
of the deceased.
Prior to July 1972, North Carolina participated in an interchange
program whereby photocopies of non-resident birth and death certificates
were sent to the National Center for Health Statistics which acted as a
broker service. In July 1972, as a result of a resolution passed by the
American Association of Vital Records and Public Health Statistics (AAVRPHS),
North Carolina joined with other states in a direct interchange of non-resident
records. The records are sent to the state of residence within
two months of the date of occurrence. The National Center for Health
Statistics acts as a control for this interchange through a report from
each state which lists the number of each type of vital record sent to
ewery other state.
Occurrence Allocation
Items 7 and 16 of the tables allocate live births and deaths
respectively to place of occurrence regardless of place of residence.
Marriage and divorce data are compiled by place of occurrence—county of
license for marriages and county of decree for divorces and annulments.
Marriage and divorce statistics are not available for cities.

Race or Color Allocation
Two broad categories, "white" and "nonwhite," are used in this
report. Nonwhite is predominantly Negro (95%) in North Carolina (2).
For births and fetal deaths, where one parent is classified as white and
the other nonwhite, the child is classified as nonwhite.

V. POPULATION BASES
Population bases for the five-year rates are averages of population
bases applicable to July 1 of each of the years 1970-1974. Methodologies for
the annual bases are as follows:
1970: Straight-line extrapolations of the 1960
and 1970 Censuses specific for race, sex
and 10-year age categories (1, 2), i.e.,
the April 1, 1970 Census counts advanced
to July 1, 1970;
1971-1974: Modified ratio-projections (6) based on
the 1960 and 1970 Censuses specific for
race, sex and five-year age categories
(1, 2) with county projections subse-quently
adjusted to sum to independent
county estimates (7-9); city projections
are not so adjusted.
The 1971-1974 population bases were provided by the Office of State
Planning of the North Carolina Department of Administration. They are derived
from a ratio-projection procedure developed by Hamilton and Perry and modified
by the Research Triangle Institute. Details concerning the projection program,
its operation and options are given by Benrud (6).
Persons interested in trend analyses of vital statistics rates should
note that the population bases described above are not the arithmetic straight-line
series used in North Carolina vital statistics reports in years prior to
1973. While it was felt that availability of more accurate methods precluded
our continuing use of the straight-line procedure, the change in series may
result in some hardship in terms of trend analysis, verily as the different

methodologies produce widely disparate population bases in some cases. Some
population bases and the corresponding rates are minimally affected by the
change; for example, the 1971 and 1972 total and race-specific rates for the
State differ by no more than one-tenth per 1,000 when rates based on straight-line
projections are compared to rates based on the new population bases for
these years. For some smaller population sectors, however, rates based on
the two different population series vary to a much larger extent. Thus, in
order to achieve comparability in rates throughout the decade of the seventies,
persons involved in trend analysis should re-compute rates for 1971 and 1972.
The age-race-sex-specific population bases required for these computations are
available in published form for the State and for each county (10); population
projections for the thirty-eight cities included in this report can also be
provided (11, 12).

VI. RATES
Computation
The relative frequency with which a vital event occurs is
expressed in the rates computed in this report. Rates are simple ratios
of two numbers which represent population enumerations for a given time
period. For annual rates, the numerators in these ratios are vital event
enumerations and the denominators represent related vital event enumera-tions
or population projections for the calendar year. For the five-year
rates, the numerators in the ratios are the average numbers of vital event
enumerations and the denominators are the average numbers of related vital
event enumerations or the average population bases for the five-year period.
In some cases, the numerators are proper subsets of the denominator; usually
these rates are expressed as percentages. Other rates are expressed in
terms of 1,000 related events of population. For example, the crude birth
rate is expressed as "the number of live births per 1,000 population."
Careful examination of the definitions of the rates computed in
this report will reveal that many are true proportions whose decimal has
been moved two or three places to the right for convenience of expression.
For example, the "premature rate" and "illegitimate rate" are true propor-tions
expressed as percentages. Other rates may not represent true pro-portions.
For example, the crude death rate and the age-specific death
rates may not, although these rates may be satisfactory estimates of
empirical or a posteriori probabilities when the decimal is moved three

places to the left. The reason the death rates may not represent true
proportions is that the denominators are population bases applicable to
July 1 of each year and hence do not represent the true population exposed
to the risk of dying during the entire year. On the other hand, the rate
of natural increase (when divided by 1,000) is neither a true proportion
nor does it represent an estimate of either a true proportion or an empir-ical
probability.
Rates are rounded to one decimal place for publication. Except
for marriage and divorce events for which race-specific rates are not cal-culated,
a blank in the rate column indicates that both the numerator and
the denominator are zero. A rate of 0.0 is shown for rates less than 0.05,
Interpretation
Inaccurate reporting, underregistration and missing values have
been discussed (Section IV). These factors affect the validity of items
of information involved in rate computation. The user of vital statistics
measures should be aware of these possible limitations.
One should interpret with caution rates which are based on small
frequencies since sampling error may be large. In this regard, Grove and
Hetzel (13) point out:
Whether or not a rate is statistically reliable
in terms of its sampling error depends upon the pur-pose
for which it is to be used. A large sampling
error may not be serious where the rates to be com-pared
differ greatly. However, if the differences
among the rates are small, only small sampling errors
can be permitted.

Due to the fact that small numbers of events may be associated
with relatively large chance fluctuations in rates, five-year rates are
included to add stability to those annual rates which involve small numbers.
Based on an increased number of events, these average annual rates are
associated with reductions in possible error.
In comparing rates, one must carefully examine the various factors
to which a difference in rates might be attributed. Unless the populations
are equivalent in all factors except the one for which the comparison is made,
conclusions about differences in rates may not be clearly drawn. Age-adjusted
rates or other further analyses may be necessary in order to make valid
comparisons.

Formulas given in this section represent computing procedures for
the rates of this report. Formulas for the annual rates are of the form

5. Premature Births
In this report, a premature birth is defined as a live born infant
weighing 2500 grams (5 pounds, 8 ounces) or less at birth, regardless of the
period of gestation. (Birth weight index recommended by the Expert Group on
Prematurity of the World Health Organization, 1950.)
The "premature rate" is the percentage of total resident live
births which were premature. Missing values (not stated birth weights) are

6. Births Out of Wedlock

never been legally married or who has been widowed or legally divorced
from her husband in excess of 280 days (14). The "illegitimate rate"

9. Fetal Deaths

duration of pregnancy, as indicated by the fact that after such expulsion
or extraction the fetus does not breathe or show any evidence of life, such

in 1950.) Consistent with North Carolina law, this report includes only
fetal deaths which do not qualify as therapeutic abortions and which result
from pregnancies of 20 or more weeks duration (see Section III).
The fetal mortality ratio (not calculated) is the number of fetal
deaths per 1000 live births. The rate computed in the tables is the number
of resident fetal deaths per 1000 resident deliveries (live births plus
fetal deaths).

14. Age at Death
Age-specific death rates are based on reported age in completed
years as of the last birthday. Errors in reporting are believed to include
underestimates for ages 22 through middle age and overestimates for persons
nearing their 21st and 65th birthdays and for those over age 65. Prefer-ences
for ages ending in certain digits (e.g., ages ending in and 5) may
also result in undue concentrations of reported ages (15).
In this report, age-specific death rates are calculated for 10-
year age intervals. The age-specific death rates are resident deaths in

17. Marriages
North Carolina law defines marriage as the legal union of a male

18. Divorces and Annulments
Legal divorce or annulment can occur only by decree of an
authorized court. Annulments, which void marriage from the beginning,
constitute less than one percent of the sum of these events. A divorce
from bed and board is a judicial separation suspending cohabitation but
not otherwise affecting the marriage bond. Divorces from bed and board
are not included in this report.
Due to a large number of cases in which race is not stated,
frequencies and rates are shown only for the total. The divorce rate

NET MIGRATION is the difference between the 1970 Census (2) and
