Dear Sir:—I have the honor to submit the First Tabulated Report of
deaths occurring in North Carolina under the Vital Statistics Law for 1914;
also a table showing total number of births, marriages and deaths with the
rate per 1,000 population, together with an outline of the work accomplished
by this Bureau.

Dear Doctor:—In accordance with your request, permit me to submit
herewith a condensed statement of the work done in this Department for the
year 1914, together with tabulated tables for counties and towns of 1,000 and
over in population which were not completed in time to be included in the
Biennial Report.
The increase in the number of certificates received is most gratifying. So
marked was this advance that on receipt of transcripts for the towns in the
registration area to August 31st, the Bureau of the Census commented on it
and the shape in which the work was gotten up.
From the attached statement it will be seen that without the earnest co-operation
of our office force the getting out of this volume of work would
have been impossible. They have labored hard and I wish them to have due
credit for their interest in the work.

Ten tables set up for towns having a population of 1,000 and over for
1913, eight tables for the 100 counties and two tables for towns of
over 1,000 in population for 1914. A number of other tables giving
advanced information.
New form of check and recapitulation sheet made showing detailed
monthly returns.
Physicians and midwives record book, showing individual monthly
returns.
Cross indices made of Local Registrars, with their deputies, physi-cians,
midwives and undertakers—8,500 names.
Complete record is kept with undertakers and casket dealers, their
sales being checked against returns.
Number of multigraph letters 62,933
Number of envelopes addressed 66,000
Number of packages of supplies 19,980
Double postals sent to mothers, requesting infants' names 11,750
Delayed infants' names entered on certificate (approx.) 9,000
Single postals to undertakers and local registrars 3,550
Supplementals received and entered in birth volumes 12,500
Complimentary calendars sent to doctors, undertakers, local regis-trars
and midwives 4,000
Corrections sent out for death certificates and returns entered in
